(P-557) Survival Analysis of ICU Patients Administered Human Albumin Infusion from the MIMIC III v1.4 Database

Background: MIMIC-III v1.4 is a large, freely-available database comprising de-identified health-related data associated with over forty thousand patients who stayed in critical care units of the Beth Israel Deaconess Medical Center between 2001 and 2012. The database includes information such as demographics, vital sign measurements made at the bedside, laboratory test results, procedures, medications, caregiver notes, imaging reports, and mortality.

Objectives: To evaluate human albumin infusion effect on survival in ICU patients from the MIMIC III v1.4 database

Methods: The database was queried for human albumin prescriptions, then for another equal number of control cases with no human albumin prescriptions selected randomly from the database. Length of stay and discharge status as either alive or dead were collected. Data were analyzed using IBM SPSS version 27 to calculate Kaplan-Meier estimates for comparing survival between the two groups.

Results: We identified 4977 records of patients prescribed human albumin, and another 4977 records of patients not prescribed human albumin randomly selected to be included in the analysis. 1061 (21.32%) death cases occurred in the human albumin group Vs 513 (10.31%) in the control group. The median survival for patients receiving human albumin was 28 days, while the median survival for patients not receiving human albumin was not reached. (log-rank test, p=0.005)
Human albumin infusion was also associated with longer length of stay with a mean of 9.2 days Vs 5.5 days in the control group. (independent samples t-test, p<0.001)

Conclusions: Human albumin administration to ICU patients showed increased mortality and longer length of stay as demonstrated by the data obtained from the MIMIC III v1.4 database.
